Social Worker - Home Health

Position Information The Social Worker is assigned to specific clinical care units and/or specific patient populations to provide services to patients and families whose medical psychological and/or social situation impacts compliance with the physician care plan the provision of appropriate health care services for the patient and/or timely discharge. The Social Worker coordinates the assistance necessary to meet patient specific needs relative to placement basic living resources home health resources therapeutic counseling and post hospital support in order to optimize outcomes. The Social Worker works collaboratively with the Discharge planner and other patient care team members. The Social Worker serves as an advocate to ensure that the patient\'s physical psychological and social needs are identified and fulfilled on a timely basis and that rights are respected and addressed.
