Job Overview Reporting to the Program Manager, the Case Manager delivers high‑quality, client‑centered case management services for individuals and families experiencing homelessness. You will manage a caseload of 15–20 participants, serving as their central point of contact. Key Responsibilities Conduct initial and quarterly psychosocial assessments. Collaborate with participants on individualized service plans and housing stability goals. Build trusted, professional relationships with clients. Provide outreach, housing navigation, and ongoing support. Facilitate referrals to healthcare, mental health, substance use, and community services. Support clients in obtaining employment, benefits, and education opportunities. Accompany clients to key appointments as needed. Use a harm reduction, strength‑based, and client‑focused approach.
