Nurse Practitioner / Physician Assistant

Position Information Location: Morgan Hill, CA. This is a W‑2 role available part‑time (16hrs per week) or full‑time (24hrs per week). Benefits Health insurance: Medical, Vision, Dental, Short‑Term Disability, Life Insurance, Critical Illness Generous PTO package Flexible hours (approximately 9AM–4PM) Competitive salary and generous bonus Continuous education reimbursement ($400‑$600) and training opportunities Full‑time virtual scribe assistance ($1,368/month value) Opportunity to earn a minimum of 33 CEUs at no cost Access to CRC’s Physiatry certification and CEU credits 401(k) plan with 2‑3% annual contribution 100% malpractice insurance coverage Company laptop Responsibilities Provide specialized PM&R/Physiatry services as a rehab consultant Represent CRC in the facility as a clinical leader, collaborating with interdisciplinary teams Assist PCP teams in managing common SNF conditions (CVA, progressive neurological, postoperative orthopedic) Collaborate with PT, OT, SLP teams to improve patient function and quality of life Track therapy patients' progress Identify and manage barriers to therapy (pain, spasticity, neurogenic bowel/bladder, etc.) Attend multidisciplinary meetings, bridging therapy and internal medicine Engage in culture building and ongoing education (didactic lectures, virtual meetings, regional dinners) Ideal CRC Clinician Passion for post‑acute healthcare and serving the geriatric population Experience in SNF or post‑acute settings (preferred) Autonomous and confident in independent practice Commitment to continuous improvement and openness to change Strong interpersonal skills and ability to understand facility partners’ needs Empathy and compassion Investment in company culture Qualifications Nurse Practitioner/APRN/Physician Assistant license California state license U.S. work eligibility Experience in post‑acute space preferred Positive attitude and dedication to delivering optimal patient care Pay Part‑time (16hrs per week): $85 per hour Full‑time (24hrs per week): $75,000–$95,000 annually Schedule 8‑hour shift with flexibility, roughly 9AM–4PM 2–3day work week; flexible days within Monday‑Friday, selectable at onboarding Supplemental Pay Types Bonus pay Commute/Relocation Must be able to reliably commute to Morgan Hill, CA, or plan to relocate before starting work.
