Powerback Rehabilitation is seeking a Travel Physical Therapist for a 13-week assignment at Hathaway Manor Extended Care in Fairhaven, Massachusetts. This SNF opportunity offers the chance to work with a diverse adult and geriatric population while helping patients regain mobility, independence, and quality of life.

As a member of the rehabilitation team, you'll evaluate patients, develop individualized treatment plans, and collaborate with interdisciplinary staff to maximize functional outcomes and support safe discharge planning.

• Perform PT evaluations and reassessments

• Collaborate with nursing, physicians, and therapy team members
• Complete timely documentation and progress reports
• Assist with discharge planning and patient/caregiver education
