Description

• Conduct thorough evaluations of patients in inpatient and outpatient environments, crafting tailored treatment plans that meet individual needs.
• Implement and oversee physical therapy interventions aimed at improving mobility, strength, balance, and overall functional capabilities.
• Team up with nurses, doctors, case managers, and other rehabilitation professionals to ensure the best possible patient outcomes.
• Educate patients and their families about therapeutic objectives, discharge processes, and at-home exercise routines.
• Maintain comprehensive documentation of evaluations, interventions, and progress in accordance with regulatory standards.
• Engage in care team discussions and interdisciplinary meetings to promote seamless care delivery.

Education

• A Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution.

Certifications

• Active state licensure as a Physical Therapist
• CPR certification preferred but not essential.
• Having specialized certifications (e.g., in neurology or geriatrics) is advantageous but not mandatory.
