Hospice RN Case Manager: Home Health Care Coordination

Optum

Amedisys, part of the Optum family, is hiring an RN Case Manager to coordinate compassionate hospice care by assessing patients and partnering with the interdisciplinary team. Kent County coverage with orientation at the Milford care center. You will develop care plans, document progress, teach families, and ensure smooth on-call coverage. Current RN license, CPR, and 1+ year nursing experience required; valid driver's license and reliable transportation necessary. #J-18808-Ljbffr
