Behavioral Health Technician - Probation & ReEntry Services (7840)

Position Summary The Behavioral Health Technician (BHT) for Bridging the Gap – Re-Entry program provides direct care services: intakes, assessments, and coordinates linkages into necessary community services and connections to providers. The BHT coordinates member referrals, provides case‑management services as needed, completes tasks for members enrolled in the Bridging the Gap Re-Entry /TIP Justice program, and liaises with Program staff, the Program Supervisor, Director, and medical records staff. The BHT delivers health and wellness and recovery‑oriented services to members, participates in vital program evaluation procedures, and keeps accurate, professional records in required database platforms. This position reports to the BTG Reentry Program Supervisor. Essential Duties Provide direct, face‑to‑face community‑based services to assist clients re‑entering the community from jail and prison, ensuring linkage into community services and resources. Offer in‑reach into jail, prison, sober‑living housing, and treatment programs to engage members in BTG Re‑Entry / TIP services and orient potential clients to the program. Consistently meet Service Delivery expectations, including $2150 weekly productivity, compliance, and timely filing (notes completed by midnight the same day). Provide transportation for members as appropriate and medically necessary, determined by the Re‑Entry/TIP team, in a personal vehicle or in a Terros Health–provided vehicle when available. Ensure ongoing collaboration, including communication of appropriate clinical information with other involved parties. Coordinate care with a member’s family, behavioral and general medical providers, community resources, and other supports. Assist members with connections to physical health services, including primary care services and medical appointment follow‑up. Provide excellent customer service to members, internal and external provider agencies, state entities, and other stakeholders (e.g., probation officers, justice liaisons, advocates, family members, residential staff, hospital staff, vocational rehabilitation, etc.) as needed. Offer direct care wellness management and recovery strategies using strength‑based engagement and psycho‑social/educational/rehabilitation services as needed. Qualifications High school diploma or GED, plus a combination of behavioral health education and work experience totaling a minimum of three (3) years; or an associate’s degree in any field, plus two years of work experience in a behavioral health service delivery. A bachelor’s degree with one year experience or a master’s degree in a related behavioral health field is acceptable. Customer service skills and excellent oral and written communication skills. Ability to work independently, with flexibility, and within prescribed deadlines. Proficiency with computer‑based systems including electronic health records, Microsoft Outlook, and other Microsoft Office products. Valid Arizona driver’s license, 21 years of age, with a minimum of two years driving experience, and compliance with Terros Health’s driving policy. Valid Arizona fingerprint clearance card or application of an Arizona fingerprint clearance card (Level1) within seven working days of assuming the role. Pass a TB test.
