RN - Case Manager

Case ManagerWe are seeking a dedicated Case Manager with acute care experience to join our team. The ideal candidate will work every other weekend and must have a minimum of two years of relevant experience. This position requires flexibility and the ability to float between like units based on need.Key ResponsibilitiesManage patient cases from admission through discharge to ensure optimal care outcomes.Collaborate with healthcare professionals to develop and implement patient care plans.Conduct assessments and facilitate communication among patients, families, and care teams.Document patient progress and maintain accurate records using Epic charting.QualificationsMinimum of 2 years of acute care experience required.First-time travelers are accepted with Clinical's approval.Availability to work every other weekend is mandatory.Strong communication and organizational skills.Additional InformationCall is not required for this position.Local candidates are welcome to apply.All requested time off (RTO) should be submitted at the time of application.Free parking is available on the first day, with assignment parking provided during orientation.Support for housing is available, including assistance from realtors if needed.About the LocationThis position is located in a city known for its proximity to seven different state and national parks, offering opportunities for outdoor enthusiasts.
