Physical Therapist (Palestine)

Christus Home Care is hiring a full‑time Physical Therapist to join our passionate team in Palestine, TX. Position Overview As a Physical Therapist in Home Health, you will complete assessments and evaluations of patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. You will develop treatment plans, perform interventions aimed at improving and enhancing the patient's well‑being, and evaluate progress. Primary Responsibilities Evaluate the patient’s functional status and physical therapy needs; assist the physician in developing the therapy plan of care. Provide services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with other health‑care team members. Observe and report the patient’s response to treatment and any changes in his/her condition to the supervising nurse and/or physician. Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice. Instruct and inform participating health‑care team members, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals. Visit patients according to Plan of Treatment, complete a progress note for each visit, and submit progress notes per LHC policy. Complete all patient evaluations and develop the PT plan of care within state‑specific guidelines. Report outcomes of evaluation, goals, and anticipated projected frequency of care. Required Qualifications Current Physical Therapy licensure in the state of practice. Current CPR certification. Current driver’s license and vehicle insurance, or reliable transportation. Ability to function in any home situation regardless of client characteristics. Compensation & Benefits Annual total cash target pay: $82,048 – $123,072. Hourly pay range: $39.45 – $59.17. Per‑visit point pay range: $47.34 – $71.00. Comprehensive benefits package, incentive and recognition programs, and 401(k) contribution (subject to eligibility).
