In addition to the support and collaboration of a full team of healthcare professionals, you will have autonomy and flexibility in scheduling. Work Location:\u202fCombination of\u202fAveanna Sherman office and Patient's Homes, with occasional home office flexibility

Compensation:\u202f$80,000 to $85,000/YR salary plus quarterly incentive potential, cell\u202fphone and\u202fmileage reimbursement\u00a0

Plus a $5,000 Sign-On Bonus

Responsibilities of the RN Case Manager:\u202f\u00a0
  • Promote\u202fand enhance\u202fthe level of clinical expertise of staff to provide safe\u202fand\u202fquality nursing care\u202ffor our patients\u202f\u202f\u00a0
  • Review physicians\u2019 orders and update the plans of care and documentation, as necessary\u202f\u202f\u00a0
  • Assist with Interviewing, selecting, training and validation of new clinical staff\u202f\u202f\u00a0
  • Participating in the release of our patients from the facility to their\u202fhome\u202fthrough\u202f\u201cStart of Care Assessments\u201d\u202fand\u202f\u201cRe-evaluations\u201d\u202f\u00a0
  • Collaborating with the operations and business development team\u202f\u202f\u00a0
  • Internal case management, evaluation and\u202fdevelopment of nursing\u202ftalent\u202f\u202f\u00a0
  • Ensure adherence to State, Federal, Local and\u00a0Aveanna\u00a0policies and procedures\u202f\u202f\u00a0
  • Quality assurance of clinical documentation
  • Care coordination with ancillary\u00a0healthcare providers
Qualifications of the RN Case Manager:\u202f\u202f\u00a0
  • Must have good standing license (RN) in the state in which the clinician will practice\u202f\u202f\u00a0
  • Must have at least 2 years of hands-on RN nursing experience\u202fwithin the last 5 years
  • Must be able to travel to patients\u2019 homes in designated territory\u202f\u202f\u00a0
  • Must have reliable transportation,\u202fvalid driver\u2019s license,\u202fand\u202fpass MVR check\u202f\u00a0
  • Current CPR certification\u202ffrom AHA\u00a0
  • Must be willing to supervise nurses providing in home skilled nursing care to infants, children, adolescents and/or adults\u202f\u202f\u00a0
