Advanced Practice ProviderRWJBarnabas Health, Primary Care Service Live is seeking a Full-Time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to be part of the primary care service line providing coverage to southern NJ.Location: Barnabas Health Medical GroupDepartment Name: Primary Care APP Float Pool (South)Shift: Full Time Days, Monday-FridayPay Range: $125,000.00 - $195,000.00 per yearQualifications:All ApplicantsCDS and DEABLSMaster's degree required or Graduate Level CertificationAPNsCurrent NJ APNNurse Practitioner CertificationRN LicensePAsPA licenseNCCPAEssential Functions:Performs comprehensive initial and episodic health assessments; in order to identify existing or potential health problemsInitiates and implements an individualized nursing and medical plan of care in collaboration with members of interdisciplinary teamInitiates appropriate diagnostic, therapeutic, medical and nursing actions in conjunction with physicianProvides follow-up care including acute and routine examinationsAssesses progress, identify problems, initiate preventive measures, recognize emergent problems, initiate proper care in collaboration with physicianMonitors and adjusts patients medication according to protocol or medical plan of careImplements nutritional therapies, as appropriateTriages patient problem calls which requires medical attentionOrders appropriate laboratory and diagnostic proceduresRecognizes normal and abnormal laboratory findings and the pathology involvedDocuments information in patients charts following documentation standardProvides health education to the patient and family (e.g., management of side effects, anticipatory teaching)Manages medical emergencies as indicated, and according to standing ordersProvides monitoring and continuity of care between health care settingsProvides patient and family counseling and reassurance in regard to psychological and or psychosocial issuesInitiates proper referrals to private or community agenciesActs as a resource and teacher for other members of the health care teamShares knowledge and expertise with staffCollaborates with the nursing and medical staff at partner and affiliate sites to coordinate continuity of careEvaluates the quality of care as well as the quality of life for patients through assessment of patient response and recommend interventions, as indicatedTravel Requirement:Employee will be expected to provide both on-site and telemedicine support and coverage for practices as neededProvides coverage for all practice sites within the assigned regional territoryMileage reimbursement will be provided for travel to assigned locations from primary departmentFrequency of travel will be contingent on the practice need for the assigned locationDuration of coverage will be contingent on the practice need for the assigned location
