Our organization operates Skilled Nursing Facilities (SNFs) that offer:

• Long-term care: Support for residents needing ongoing skilled assistance and compassionate daily care.
• Short-term, sub-acute rehabilitation: Services for patients recovering from surgery, illness, or injury.
• Collaborative care: An interdisciplinary team approach involving nursing, therapy, and medical staff working in unison.
• Diverse patient population: Care for individuals with orthopedic, neurological, cardiopulmonary, and post-acute needs.

The patient demographic may vary slightly by facility. As a therapy professional, you will play a crucial role in helping patients achieve functional improvements and enhance their quality of life in a supportive, patient-focused setting.

Position Overview

The Physical Therapist (PT) delivers direct rehabilitation services in physical therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.

Education, Experience, and Licensure Requirements

• Must be a graduate of an accredited physical therapy program (BSPT, MSPT, or DPT); those graduating in 2003 or later must possess an MSPT or DPT.
• Current registration/licensure in the applicable state is required, with maintenance of an active license in good standing throughout employment.
• Preferred: At least one year of experience in post-acute care or a related field.
• CPR certification is required upon hire or must be obtained during orientation; certification must remain current during employment.

Specific Job Responsibilities

• Engage in community health initiatives and projects as appropriate.
• Proficiently use Microsoft Word, Excel, and email.
• Coordinate with patients, families, and support departments to effectively address patient needs.
• Exhibit proper body mechanics consistently.
• Make independent decisions when necessary.
• Understand and comply with practices, procedures, and regulations governing operations in the post-acute care setting.
• Implement and interpret departmental programs, goals, objectives, policies, and procedures.
• Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation.
• Maintain professional relationships with colleagues, vendors, and others.
• Ensure confidentiality of all proprietary and sensitive information.
• Adhere to organizational policies, including those related to harassment and compliance.
• Exhibit integrity and professionalism by following the organization's Code of Conduct and completing mandatory compliance training.

Essential Functions

• Implement physical therapy treatment plans for patients.
• Set, evaluate, and adjust realistic, measurable, and functional goals.
• Supervise and assess care provided by PTAs, Rehab Aides, and PT and PTA students.
• Document care accurately and in a timely manner.
• Utilize therapy software effectively and accurately.
• Provide excellent customer service and maintain a positive attitude towards patients.
• Assist in the safe evacuation of patients when necessary.
• Demonstrate reliable and regular attendance.
• Employ reasoning skills and sound judgment in decision-making.
• Communicate effectively and collaborate within an interdisciplinary team.
• Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, walking, and reaching throughout working hours.
• Read, write, speak, and comprehend the English language.
