Full-Time Physical Therapist (PT) LeRoy, NY | Full-Time | W-2 or 1099 Position Overview We are seeking a motivated and licensed Physical Therapist (PT) to join our team full‑time, providing care both at our facility in LeRoy, NY, and to patients in the surrounding community. The ideal candidate is clinically versatile, comfortable transitioning between facility and community-based settings, and committed to helping patients achieve their highest level of functional independence. Key Responsibilities Provide physical therapy services to patients at our LeRoy, NY facility and in community settings in the surrounding area. Evaluate, diagnose, and develop individualized treatment plans addressing mobility, strength, balance, pain management, and functional rehabilitation. Implement and progress treatment plans in accordance with physician orders and patient goals. Maintain accurate, timely clinical documentation in compliance with facility, community care, and regulatory standards. Collaborate with an interdisciplinary team including physicians, speech‑language pathologists, occupational therapists, and nursing staff. Provide patient and caregiver education to support recovery and functional independence. Conduct home and community environment assessments to inform treatment planning as needed. Participate in care planning meetings and contribute to quality improvement initiatives. Adhere to all applicable federal, state, and facility compliance standards. Qualifications Doctor of Physical Therapy (DPT) from an accredited program is required. Current New York State licensure as a Physical Therapist (required). Experience in a skilled nursing facility, outpatient, or community‑based setting preferred. Ability to manage a caseload across both facility and community environments. Reliable transportation and a valid driver's license are required for community visits. Strong clinical documentation skills and familiarity with electronic health records (EHR). Excellent interpersonal, organizational, and communication skills. Compensation Competitive salary or tiered pay‑per‑visit commensurate with experience | W‑2 or 1099 Employment.
