Facility: Sullivan County Health Care
Position: Licensed Practical Nurse (LPN) – Long-Term Care
Shift: Evenings
Contract: 13 Weeks
Schedule: 48 Hours/Week
Estimated Weekly Gross: $2,342

Position Overview

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join the Long-Term Care team at Sullivan County Health Care in Claremont, New Hampshire. This is an excellent opportunity for an experienced LPN who is passionate about providing quality, patient-centered care to residents in a supportive LTC environment.

Key Responsibilities

• Provide safe, compassionate nursing care to long-term care residents.

• Administer medications and treatments according to established protocols.

• Monitor residents' conditions and promptly report changes.

• Perform assessments and maintain accurate clinical documentation.

• Collaborate with RNs, physicians, CNAs, and other members of the healthcare team.

• Assist with activities of daily living and promote resident comfort and dignity.

• Follow infection-control, safety, and facility policies.

• Build positive relationships with residents and their families.

Ideal Candidate

• Active LPN license

• Previous Long-Term Care/SNF experience preferred .

• Strong clinical assessment and communication skills.

• Reliable, compassionate, and team-oriented.

• Ability to work evening shifts and commit to a 48-hour weekly schedule .
