Field-Based Community Health Advocate

Taleo is seeking a Field Based Community Health Worker to identify and initiate referrals for social services, support members with home and social resources, and educate on resources in the El Paso area. This full-time role focuses on addressing social determinants of health, coordinating with care teams, and ensuring timely, quality care for Medicaid members in the StarPlus program.
