Minimum Qualifications: Graduate from an accredited school of Practical Nursing in the State of Illinois and current licensed as a Practical Nurse in the State of Illinois. BLS certification Successful completion of coursework for medication administration (Pharmacology) as a Practical Nurse in accordance with the Illinois Department of Financial and Professional Regulation (IDFPR) regarding Standards of Pharmacology/Administration for Medication Course for Practical Nurses (Must provide documentation if requested)
Preferred Qualifications: Two (2) years recent full time paid clinical experience in an acute care health facility or ambulatory care facility
Call Requirements: N/A
Weekend Requirement: Every other weekend/ 2 weekends per month
Will clinician float within scope to meet facility needs? Cook is a MANDATORY FLOAT FACILITY, all resources are required to float at the facilities discretion.

Clinicians should expect to float regularly. There are no guaranteed home units. Clinicians can be floated across a variety of units, including all units within their specialty, which is common, and at times to areas outside their primary competency. If floated outside of their primary competency, a team-based care model will be implemented to ensure appropriate support. Cook may request a clinician to float outside of their home facility, provided the distance does not exceed 50 miles. Travelers may be notified of a float assignment up to 2 hours prior to the start of their shift. Refusal to float may result in cancellation of the assignment.
References: Two professional references required prior to start: References must be from the Candidate’s most recent employers One reference must be from the Candidate’s most recent employer. If the 2nd most recent employer is not available, the next most recent employer must be used. Only one reference per employer is acceptable International Candidates: must obtain SSN/SSC prior to submission. Cook's HR does not allow SSN/SSC to be pending at start, even with proof of application. Local rates: Resources who live within 75 miles of the facility will be considered local. Locals accepted at lower rate. Please reach out with questions. Block Scheduling: Cook does not approve block schedules or special requests. All time off must comply with the RTO policy. RTO: RTO must be included at the time of submission and follow these guidelines: 5 days max with no more than 2 consecutive days. No RTO on weekends. Holiday RTO: No holiday RTO. No RTO the day prior or after a holiday. If your traveler calls out on a holiday, the facility can cancel their contract. Returning resources: CCH requires all returning resources to undergo full recredentialing and reattend orientation. Full NBO will be applied. This is applicable even if the break in contract was less than 30 days. Doc Due Date: Compliance deadline is the Monday, the week before orientation, by close of business. Additional Compliance details: Candidate must be responsive and provide ETAs for all required items Lab work must be completed within 72 hours of accepting an assignment If unresponsive or unable to meet these expectations, the candidate may be at risk of not continuing the assignment Modules: Modules are non-billable, average 4 hours Modules are completed pre-start Any modules completed while on assignment, including LMS/annual module requirements are billable
