You assist the physician in evaluating the level of functioning in each patient by providing initial and ongoing assessments. After making decisions based on assessments, you help develop or revise the plan of care, prepare clinical and progress notes, and consult with the family and other agency personnel. You then provide the appropriate therapeutic treatments while maintaining the comfort of your patients.In addition, you continuously evaluate the effectiveness and outcomes of the care you're administering. Keeping track of your patient's condition, you review their plan of care and apply the appropriate revisions. Using your exceptional communication skills, you coordinate services with other home health care staff members to ensure that patients are receiving well-rounded treatment. At times, you supervise physical therapy assistants and other home health aides as needed as well as evaluate our equipment to ensure optimal levels of functioning.You provide all services in accordance with CMS documentation and billing requirements, including functional reassessment requirements. The safety of your patients is of the utmost importance to you, and you take great care to practice infection control by following universal precautions. You also actively recognize and report any forms of abuse according to policy as well as federal and state laws. When needed, you assist with orientation for new hires and participate in any training activities.
