Position Summary
A full-time Primary Care Physician is sought to support a growing eldercare program that emphasizes community-based care, whole-person treatment, and a collaborative interdisciplinary approach. This position focuses on serving medically complex senior patients through comprehensive in-person assessments, care coordination, and oversight of medical treatment across settings including homes, nursing facilities, and hospitals.
Key Responsibilities
- Conduct in-person comprehensive assessments for new and existing patients.
- Coordinate participant care plans in collaboration with a full Interdisciplinary Team (IDT).
- Oversee care for patients admitted to nursing homes and hospitals (including short-term, long-term, and acute placements).
- Monitor and manage transitions of care (e.g., admits and discharges).
- Guide and supervise Nurse Practitioners and other team members as needed.
- Visit participants in nursing homes as needed, based on individual care plans.
- Participate in rotating night and weekend call coverage.
- Support seamless coordination with community physicians and other specialists.
Minimum Qualifications
- Doctorate in Medicine (MD or DO) from an ACGME-accredited residency program
- Board Certification in Internal Medicine, Family Practice, or Geriatrics (preferred)
- Active California Medical License (unrestricted)
- Active DEA license and state-controlled substance registration
- Eligible for enrollment as a provider in Medicare and Medi-Cal
Preferred Attributes
- Experience working in geriatrics, palliative care, or home-based medicine
- Strong collaborative, patient-centered care philosophy
- Skilled in interdisciplinary care delivery models

- Visa candidates not considered for this position
- Interview travel is not reimbursed
