What You'll Do:

• Evaluate patients' functional abilities through various assessment measures.
• Develop individualized treatment plans based on assessment data and patient tolerance.
• Implement and document treatment regimes in coordination with physicians.
• Educate patients and families about conditions, treatment options, and prognosis.
• Supervise Physical Therapist Assistants, aides, and students as appropriate.
• Recommend adaptive/supportive equipment and assist with securing devices.
• Maintain accurate documentation compliant with regulatory and billing standards.
• Promote quality of care through interdisciplinary collaboration and community engagement.
• Participate in ongoing education, team meetings, and process improvement initiatives.
• Serve as case manager for assigned patients, ensuring comprehensive care.

What You Need:

• Valid Physical Therapist license in Michigan.
• Prior experience in outpatient setting preferred.
• Strong communication and organizational skills.
• Ability to work independently and as part of a team.
• Commitment to patient-centered care and adherence to safety protocols.
• Interpersonal skills for effective patient education and collaboration.
• Ability to lift up to 50 lbs. occasionally and perform physical functions as outlined in the job description.
• Must complete OSHA training annually and follow infection control procedures.
