Work Environment

This role requires an onsite presence, primarily within patients’ homes, allowing you to build meaningful relationships with those you serve. Key Responsibilities

• Perform thorough health assessments, including comprehensive physical examinations and screenings for a varied patient demographic, ensuring personalized care tailored to individual needs.
• Diagnose and treat both acute and chronic health conditions, employing evidence-based practices to promote optimal health and disease prevention.
• Design and implement detailed treatment plans in collaboration with patients and other healthcare providers, adapting strategies based on individual patient responses.
• Provide culturally competent care, advocating for patients’ rights and facilitating their access to necessary healthcare services.
• Coordinate necessary referrals and ensure thorough follow-up care for patients, establishing continuous patient-provider relationships.

Patient Population / Client Focus

Qualifications

• A Master’s degree in Nursing from an accredited program is required to ensure a strong foundational knowledge of advanced nursing practices.
• Current Nurse Practitioner certification, along with an active state nursing license in New York, is essential for practice in this position.

• This position offers a competitive salary range of USD 150,000 to 160,000 per year, depending on experience and qualifications.
