Position Summary

Optimal Home Care Services LLC is seeking a compassionate, dependable, and professional Female Caregiver/Direct Support Professional (DSP) to support an adult individual in Brooklyn Park, MN. The ideal candidate will assist with daily living activities, community involvement, companionship, and promoting independence in a person-centered environment.

Responsibilities

• Assist with personal care and daily living activities.
• Provide companionship and emotional support.
• Support participation in community activities and outings.
• Assist with meal preparation and healthy living routines.
• Provide transportation to appointments and community activities as needed.
• Monitor and report changes in health, behavior, or daily functioning.
• Encourage independence and achievement of personal goals.
• Complete documentation accurately and on time.
• Follow agency policies and person-centered support practices.

Qualifications

• Experience as a Caregiver, DSP, PCA, Homemaker, or related role preferred.
• Compassionate, patient, and dependable.
• Excellent communication and interpersonal skills.
• Valid driver's license and reliable transportation preferred.
• Ability to pass a DHS background study.
• CPR/First Aid certification preferred or willingness to obtain.
