Discharge Planning Assistant - Care Management- Day Shift

Job Title Discharge Planning Assistant Employment Type Full time Shift & Time Day Shift – 8:00 a.m. to 4:30 p.m. Location Germantown, Maryland Benefits Medical, Dental & Vision PTO Free Parking Metro Access Tuition Reimbursement 403(b) Responsibilities Set up transports from hospital to rehab facility using transportation services provider. Obtain prior authorization for patients being transferred to Skilled Nursing Facilities (SNF). Follow up on referrals, missing patient documentation and check bed status at SNF. Facilitate referrals to community agencies as directed by Social Worker/Case Manager via phone and web‑based applications (EPIC). Communicate with SNFs (liaison and admission teams) to facilitate appropriate patient placement considering patient/family choice, clinical condition/needs, and insurance coverage. Determine need for and obtain authorization from insurers for post‑acute care including SNF services and ambulance transportation. Qualifications High school diploma or GED required. Some college preferred or equivalent experience including knowledge of medical terminology. Minimum 1–2 years’ work experience in a health care setting preferred. Proficient with MS Office; working knowledge of Excel, typing, data entry, and transcription skills with accurate spelling and grammar. Strong analytical, organizational, and prioritization skills; ability to adapt quickly to changes and interruptions in a fast‑paced environment.
