Our client in Putnam, Connecticut is seeking a Patient Care Coordinator for a contract opportunity. This role is ideal for someone who is highly organized, compassionate, and experienced in supporting patients through scheduling, communication, and administrative coordination in a healthcare setting.

Key Responsibilities:

• Coordinate patient appointments, follow-ups, and referrals
• Serve as a primary point of contact for patients, providers, and internal staff
• Verify and update patient information in the system
• Assist with insurance verification, authorizations, and related documentation
• Handle inbound calls and respond to patient questions with professionalism and empathy
• Maintain accurate records and ensure timely documentation
• Support front-office and care coordination functions as needed

• Previous experience in a healthcare administrative, patient services, or care coordination role
• Strong communication and customer service skills
• Ability to manage multiple priorities in a fast-paced environment
• Proficiency with electronic medical records and standard office systems
• High attention to detail and strong organizational skills
• Knowledge of medical terminology and insurance processes preferred
