Seven Healthcare are seeking an experienced Physical Therapist for a contract role in Ashland, KY. This position offers an opportunity to work across multiple counties, providing critical physical therapy services in a home health setting. - Perform physical therapy evaluations and treatments based on physician orders/referrals.
- Complete comprehensive evaluations using appropriate tools and procedures.
- Review patient records for diagnosis, medical history, precautions, and contraindications.
- Establish measurable goals and develop treatment plans to advance patient independence and function.
- Re-assess treatment plans regularly and communicate progress to the treatment team.
- Consult with medical providers regarding patient evaluation, treatment, and discharge.
- Recommend and assist in securing adaptive equipment for patients.
- Maintain and submit documentation as required by regulations.
- Supervise Physical Therapist Assistants and Aides.
- Participate in staff development programs and obtain continuing education.
- KY Physical Therapist Licensure; Compact Licensure Accepted
- 1 year home health physical therapy experience
- Previous OASIS experience
- Proficiency with Home Care Home Base
