Home Health PT We have multiple openings for full‑time and part‑time Physical Therapists (PT) in ContraCostaCounty, CA to join our premier home‑health care team. The schedule is flexible and staff can set their own visits. Candidates will receive a $10,000 sign‑on bonus for full‑time PTs. About the Company Prohealth is a locally owned Home Health, Hospice & Palliative Care agency with 4 locations servicing over 12 counties in Northern California. We have an average daily census of over 1800 patients and have led the industry for over 15 years. We provide customized clinical care with a patient‑focused model that includes nursing, physical therapy, occupational therapy, speech therapy, and registered dietitians. Prohealth Home Care seeks a compassionate and dedicated Home Health Physical Therapist (PT) who wants to make a positive difference in our community by providing skilled, dignified care in patients’ homes, along with emotional support for patients and families. Job Responsibilities Assess and evaluate therapeutic, rehabilitative, and functional status; develop the plan of care and evaluate home environment. Assess muscle strength, mobility, gait, range of motion, and potential for rehabilitation. Initiate physical therapy programs and instruct family or caregivers in appropriate phases of therapy. Participate in case conferences and communicate program goals. Identify patient and caregiver needs for other home health services and refer as necessary. Qualifications Valid PT license to practice in California. Current BLS/CPR certification. Valid California driver’s license. Minimum of one year of Physical Therapy experience (required or preferred). Benefits Competitive pay. 401(k) with company match. Health care plans (medical, vision, dental).
