Job Description
Position Overview The Physical Therapist will provide comprehensive rehabilitation and physical therapy services within a correctional healthcare setting. The role is responsible for evaluating, treating, and monitoring individuals with a wide range of physical impairments and rehabilitation needs while promoting functional independence and overall well-being. The ideal candidate will possess strong clinical expertise, the ability to work independently, and experience collaborating with multidisciplinary healthcare teams to deliver high-quality patient care.

Key Responsibilities • Independently evaluate, assess, and administer physical therapy services to individuals with complex rehabilitation needs. • Develop, implement, and modify individualized treatment plans based on patient evaluations, clinical findings, and therapeutic goals. • Provide therapeutic interventions designed to improve mobility, strength, functional capacity, and overall physical health. • Monitor patient progress and adjust treatment plans as clinically appropriate to maximize rehabilitation outcomes. • Maintain accurate and timely documentation of evaluations, treatment plans, progress notes, and patient outcomes. • Collaborate with the Director of RMU, Nursing Director, Facility Health Services Director (FHSD), physicians, and other healthcare professionals regarding patient care and rehabilitation needs. • Participate in interdisciplinary care planning and treatment discussions. • Attend and actively participate in bi-weekly care conferences. • Educate patients regarding treatment plans, therapeutic exercises, injury prevention, and rehabilitation goals. • Ensure compliance with facility policies, rehabilitation standards, and applicable regulatory requirements. • Support quality improvement initiatives and contribute to the delivery of effective rehabilitative healthcare services.

Work Schedule & Conditions • Schedule: Monday - Thursday • Shift Hours: 8:00 AM - 4:00 PM EST • Four (4) days per week • Eight (8) hours per day • Overtime: Not required • Travel: Not required • Holidays: Not required to work on New York State holidays

Facility Information • Mohawk Correctional Facility is a medium-security correctional facility located in Rome, New York. • Walsh Regional Medical Unit (RMU), located within Mohawk Correctional Facility, operates as a maximum-security medical unit. • The selected candidate will provide rehabilitation services within a secure correctional healthcare environment.

Qualifications & Requirements • Licensed Physical Therapist or other licensed rehabilitation care practitioner authorized to practice in the State of New York • Experience providing rehabilitation services to patients with complex physical therapy needs preferred • Strong knowledge of physical therapy evaluation techniques, treatment planning, and rehabilitative interventions • Ability to work independently while managing a diverse patient caseload • Experience collaborating with multidisciplinary healthcare teams • Excellent communication, documentation, organizational, and interpersonal skills • Commitment to patient-centered care, rehabilitation excellence, and professional standards

Background Check Requirements • Must successfully complete all facility-required background checks and security clearances prior to assignment

Vaccination & Medical Requirements • Must meet all facility medical clearance requirements prior to assignment

Required Training • Facility-specific orientation and training as required prior to assignment
