Job Title: LVN/LPN - Home Health
Location: Jacksonville, FL
Setting: Home Health
Pay Range: $1,500-$1,700 gross per week
Schedule: 08:00AM-05:00PM
Contract Length: 13 weeks
Candidate Type: Local and traveler friendly
Requirements: Qualified applicants MUST have at least 2 years of experience in home health, have a valid LPN license and be willing to obtain FL licensure
Responsibilities:
- Provide comprehensive in-home nursing care to patients, including assessments and care planning.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Educate patients and family members on health management and treatment plans.
- Document patient progress and maintain accurate medical records.
- Ensure compliance with all state and federal healthcare regulations.
