Home Health Physical Therapist – Durham, NC

Join a well-established Home Health team providing individualized care throughout the Durham area. This assignment is ideal for a PT who enjoys autonomy, flexible scheduling, and developing meaningful one-on-one relationships with patients in the home setting.

This opportunity is with Amedisys and requires an experienced Home Health therapist who can confidently manage a caseload, complete OASIS documentation, and work efficiently in HomeCare HomeBase.

Assignment Details

• Home Health Physical Therapist
• 13-week travel contract

• Adult and geriatric patient population
• Territory-based home visits throughout the Durham area

Requirements

• Previous Home Health PT experience required
• OASIS experience required
• HomeCare HomeBase (HCHB) experience required
• Current North Carolina PT license required
• BLS certification required
• Strong independent clinical and organizational skills

Responsibilities

• Evaluate and treat patients in the home setting
• Complete OASIS assessments and point-of-care documentation
• Document daily visits using HomeCare HomeBase
• Develop individualized treatment plans focused on safety, mobility, and functional independence
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary teams
• Educate patients and families on mobility, fall prevention, and home safety
