Minimum Qualifications
Associate degree in Physical Therapy from an accredited educational program.
Active Oregon PTA license required to start the assignment.
Home health experience in Oregon required.
BLS/CPR certification required (AHA or ARC accepted).
Experience with patient subsequent visits and home health documentation required.
Valid drivers license and ability to travel between patient homes within Multnomah and Clackamas counties. Mileage reimbursement provided between visits.
Preferred: Experience with HCHB (Homecare HomeBase) electronic documentation.

Assignment Details
Facility type: Home Health.
Shift: Evenings.
Shift hours: 2000-0400.
Weekly hours: 36 guaranteed per client notes.
Coverage area: Multnomah and Clackamas counties; travel between patient homes required.
On call/weekend requirement: No.
