Behavioral Health Specialist - Remote Short-Term Therapy

Oak Street Health is seeking a Behavioral Health Specialist to provide care management and short-term evidence-based psychotherapy to patients with behavioral health needs. You will collaborate with Care Teams across primary clinics, assess patients, and coordinate referrals to our psychiatric consultant or external resources. A remote-work option may apply based on program metrics.
