References must be from assignments worked in the past 12 months. - Required certifications need to be active with nothing pending. - Locals accepted at the same base rate. - PBDS Assessment is waived. - ECG rhythm recognition test and safeMedicate will be completed during orientation, with an acceptable score required. Two attempts allowed. - Facility does not accept any traveler with an open case or any license hits. **Additional Notes:** - Nurses and CSTs are exempt from Nicotine Free Facility Requirements as of 9/30/22; this exemption is temporary. - All other travelers (aside from Nurses and CSTs) must be nicotine-free, requiring a Negative 10 Panel + Nicotine (Cotinine) drug screen within 12 months of start, updated annually. - BCI/FBI prints must be completed within 48 hours of signing the contract. - Extensions with the client are possible for a period of 18 months, with a mandatory 6-month break thereafter, unless otherwise approved. - Modules are billable up to 4 hours, except for non-billable pre-start modules. - Profiles should be submitted with a completed Standard Qualivis Cover Sheet, including interview availability in the notes section.
