You will provide comprehensive medical services, foster lasting relationships with your patients, and collaborate with a high-functioning care team to drive optimal health outcomes. Provide direct patient care through office visits, telehealth, and occasional same-day appointments Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions Educate and empower patients on preventive health and functional medicine principles Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff Document visits in the EHR in a timely and compliant manner Participate in team meetings, quality improvement initiatives, and training sessions Support clinic operations by contributing to a positive, team-oriented environment Active Oregon license as a Nurse Practitioner*, PA-C, MD, or DO ( or eligibility to obtain ) Board certification in Family Medicine, Internal Medicine, or applicable specialty Strong interest in preventive, functional, and lifestyle medicine Proficient in using electronic health records *Nurse Practitioner Licenses Accepted: FNP, Family Nurse Practitioner AGNP- PC, Adult Gerontological Nurse Practitioner (Primary Care) Medical, Dental, Vision, and Life Insurance
