POSITON TITLE: Mental Health Case Manager

Non-Exempt Position

RESPONSIBLE TO: Program Director

MAJOR DUTIES AND CORE COMPETENCIES

Under the supervision of the Program Director and Program Supervisor, the Mental Health Case Manager provides community-based behavioral health, care coordination, and re-entry support services to participants. The position requires independent judgment, effective collaboration, regulatory compliance, and a commitment to recovery-oriented, trauma-informed, and person-centered care.

Clinical Assessment and Care Planning

  1. Conduct comprehensive assessments, including whole-person and Health-Related Social Needs (HRSN) assessments, to identify participant strengths, barriers, risks, and service needs.
  2. Develop, implement, and monitor individualized Re-Entry Care Plans that are strength-based, participant-centered, culturally responsive, and aligned with identified goals and service needs.
  3. Regularly evaluate participant progress and adjust interventions and service plans to support successful community integration and recovery outcomes.

Care Coordination and Service Delivery

  1. Provide targeted re-entry case management services in accordance with Washington State's Medicaid Re-Entry Demonstration Initiative (1115 Waiver), agency standards, and clinical recommendations.
  2. Coordinate access to behavioral health treatment, substance use disorder services, medical care, housing resources, transportation, employment supports, benefits, and other community services.
  3. Facilitate incoming and outgoing referrals and ensure continuity of care across providers, agencies, and systems.
  4. Utilize sound judgment and problem-solving skills to address participant needs, barriers to care, and service engagement challenges.

Interdisciplinary Collaboration

  1. Serve as an active member of the multidisciplinary treatment team by contributing clinical observations, recommendations, and participant progress updates.
  2. Collaborate effectively with correctional facilities, healthcare providers, community agencies, managed care organizations, probation officers, courts, and other stakeholders to support participant outcomes.

Documentation and Regulatory Compliance

  1. Maintain timely, accurate, and complete documentation of all services, contacts, assessments, and care coordination activities in accordance with:
    • WAC 246-341
    • RCW 71.05
    • Medicaid Re-Entry Demonstration requirements • HIPAA and 42 CFR Part 2 confidentiality requirements
    • Agency policies and procedures
  2. Demonstrate accountability for compliance with federal, state, and agency regulations governing behavioral health service delivery.
  3. Support agency quality improvement activities, accreditation standards, and performance outcomes through accurate documentation and adherence to best practices.

Community Engagement and Resource Development

  1. Develop and maintain collaborative relationships with community agencies, service providers, and referral partners to improve participant access to resources and support services.
  2. Identify community resources and advocate for participants to obtain services necessary to support recovery, stability, and successful re-entry.
  3. Represent the agency professionally in community meetings, care coordination activities, case staffing, and other approved forums.

Professional Accountability

  1. Demonstrate professionalism, ethical decision-making, cultural humility, and adherence to agency values and professional standards.
  2. Participate in supervision, training, staff development, and continuous quality improvement activities.
  3. Support and implement agency policies, procedures, and organizational initiatives.

KNOWLEDGE, SKILLS, AND QUALIFICATIONS

Required Qualifications

  • Bachelor's degree in Behavioral Health, Psychology, Social Work, Human Services, Counseling, or a closely related field preferred.
  • Washington State Department of Health credential required or ability to obtain and maintain required credentialing.
  • Minimum of one year of paid or volunteer experience working with individuals experiencing mental health, substance use, co-occurring disorders, or related behavioral health conditions.
  • Working knowledge of behavioral health recovery principles, trauma-informed care, and participant-centered service delivery.
  • Familiarity with applicable Washington State behavioral health regulations, including RCW 71.05 and WAC 246-341.
  • Preferred Qualifications
  • Substance Use Disorder Professional Trainee (SUDPT) credential.
  • Experience providing re-entry, care coordination, case management, or community behavioral health services.

POSITION COMPLEXITY

This position serves participants with diverse backgrounds and complex behavioral health, substance use, medical, housing, legal, and social service needs. Success in this role requires strong interpersonal skills, sound judgment, adaptability, independent decision-making, and the ability to coordinate services across multiple systems while maintaining participant engagement and regulatory compliance.

 

IMMEDIATE SUPERVISOR: Program Director

Salary: $22.62 - 24.87/hr DOE
