As our census grows, PRN CNA opportunities may expand into additional hours or roles as business needs dictate . Position Summary The PRN CNA provides direct patient care under the supervision of the RN Case Manager and in accordance with the Aide Plan of Care. This role supports patient comfort, safety, and dignity while ensuring compliance with CMS Conditions of Participation and CHAP standards. Minimum Qualifications High School Diploma or GED preferred Completion of one of the CMS §418.76 competency pathways within the last 24 months Current listing in good standing on the Texas Nurse Aide Registry Hospice or elderly‑care experience preferred EMR experience preferred (Kinnser/WellSky) Valid driver’s license, auto insurance, and reliable transportation Ability to speak, read, and write in English; Spanish bilingual preferred Key Responsibilities Direct Patient Care including hygiene, grooming, toileting, and mobility assistance Plan of Care Implementation with required visit frequency Observation & Reporting of patient condition changes to RN Infection Control and maintaining a clean, safe environment Documentation in the EMR accurately and timely Communication with patients, families, and staff Safe Transfers & Ambulation Operational Support and effective time management Patient homes, assisted‑living facilities, skilled nursing facilities Exposure to varying home environments, weather conditions, and emotionally sensitive situations Possible exposure to airborne and bloodborne pathogens Requires prolonged standing, walking, and ability to lift up to 50 lbs Travel throughout Northern San Antonio and Boerne Why Join Passion Hospice CHAP‑accredited with zero deficiencies Supportive interdisciplinary team PRN flexibility with potential for expanded hours as census grows #J-18808-Ljbffr
