Position Overview

This position is responsible for coordinating and directing the provision of home nursing care according to physicians' orders, utilizing agency policies and procedures through the competent application of the nursing process. The ideal candidate will work under the supervision of the Clinical Manager and will be responsible for evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.

Key Responsibilities

• Provide home nursing care as per physician orders.
• Coordinate and direct patient care following agency policies and procedures.
• Supervise and evaluate the performance of Home Health Aides and LPNs/LVNs.
• Conduct home visits and ensure high-quality patient care delivery.
• Maintain accurate documentation of patient care in the designated system.
• Engage in periodic office meetings as required.

Travel Requirements

The role requires reliable transportation for home visits. Candidates must be willing to travel up to 75 miles from their home clinic, with mileage reimbursed at the IRS standard rate. Commute to and from the first and last patient is not billable.

Work Schedule

This is a day shift position from 8:00 AM to 5:00 PM, Monday to Friday, with no guaranteed hours. Flexibility is expected as the number of patients varies. Typically, 5-10 patients will be assigned per day, equating to approximately 6-8 points in productivity, with a total expectation of 30 points per week.

Qualifications

• Minimum of one year of adult home health experience or one year of hospice experience.
• Recent experience in the relevant specialty is required.
• Certifications required: CPR and BLS.
• Proficiency in HomeCare HomeBase (HCHB) is mandatory, with a requirement of having used it within the last six months.
• Proficiency in OASIS is also required for RNs, PTs, and OTs.

Patient Care Expectations

• Average number of patients per day: 5-8.
• Weekly productivity expectations: 30 points, translating to approximately 40 hours of work.
• Documentation must be completed within 24 hours of patient visits.

Location

This position covers the following areas: Cambridge, Somerville, and Watertown in Massachusetts.

Additional Information

Candidates must possess a valid driver's license and auto insurance. This role requires acceptance of all patients on a daily schedule, regardless of patient distance, and there are no guaranteed hours with the contract. If there are insufficient patients, there may be opportunities to float to nearby clinics for additional hours.
