Flat Irons Flex Care
Key Job Responsibilities: Conduct initial and annual in-home assessments, as well as reassessments on an as needed basis. Create and maintain individualized care plans that outline the services needed by the client. Oversee the delivery of IHSS/Agency-based services to clients, ensuring they are updated to reflect current client needs. Maintain accurate, timely documentation in compliance with IHSS and Medicaid regulations. Collaborate with the client or authorized representative to troubleshoot any concerns related to care plans. Requirements: Current Registered Nurse (RN) license that is active in Colorado Pass Colorado Bureau of Investigation (CBI) and Colorado Adult Protective Services (CAPS) background checks Ability to work independently and travel to client homes Experience in home care, home health, or community health nursing preferred. #J-18808-Ljbffr
