Responsibilities The Physical Therapist will primarily provide outpatient rehabilitation services in an in‑home setting for the older adult population. The in‑home setting could be a patient’s private residence or a patient’s room in an independent living facility. Powerback Rehabilitation To You allows the Physical Therapist to evaluate and treat in the most functional environment which is the patient’s home. Assesses and develops physical therapy programs to facilitate rehabilitation. Identifies needs and makes referrals to other disciplines. Implements individualized therapy programs designed to restore, reinforce, and enhance performance. Instructs, educates and trains patients and caregivers to promote independence and productivity. Directs activities to promote and maintain health. Provides a flexible schedule and work environment in various settings. Supervises Physical Therapy Assistants, temporary licensees, students and support personnel as required. Ensures adherence to department and patient treatment schedule, timely clinical and billing documentation, and work‑related meetings. Attends patient care, staffing conferences and other related meetings. Consults with rehabilitation team members to select appropriate therapeutic regimens. Adapts program and/or plan of care according to the needs of each patient. Promotes a safe environment and enforces elimination of fire and safety hazards. Orders supplies and equipment as necessary. Cleans, maintains, identifies, and reports needed equipment repairs. Puts patient service first: ensures patients and families receive the highest quality of service. Scans, examines, and evaluates patients including history, systems review, and tests to establish diagnosis, identify impairments, and design a plan of care. Develops treatment goals, methods, and completes all related documentation. Applies therapeutic techniques such as therapeutic exercise, functional training, manual therapy, airway clearance, integumentary repair, electrotherapy, and health education. Designs, fabricates, fits, and trains patients in adaptive devices including orthotics and prosthetics. Selects appropriate activities suited to the individual’s physical and mental capacity. Performs routine reexaminations to modify or progress plan of treatment. Incorporates health and wellness programming into treatment plans. Conducts home and community mobility assessments and provides recommendations. Performs other related duties as required. Qualifications Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility. Master’s degree in Physical Therapy, or Master’s degree in a related field plus a Bachelor’s degree in Physical Therapy, or Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience. Degree earned outside the U.S. must be evaluated as the U.S. equivalent. Valid driver’s license or reliable, independent transportation to and from patients’ residence.
