Physical Therapist

Overview Blue Stone Therapy seeks a Physical Therapist (PT) to provide rehabilitation services to adult and geriatric patients at Lyons Specialty Care in Rock Rapids, IA. The role focuses on evaluating and treating patients in a skilled nursing and post-acute rehabilitation setting, with travel to partner communities such as Sibley, IA and Le Mars, IA as required. Responsibilities Perform physical therapy evaluations and develop individualized plans of care. Provide skilled physical therapy treatment focused on functional and measurable outcomes. Address gait training, balance, mobility, strength, range of motion, pain, and discharge planning. Supervise Physical Therapy Assistants in accordance with licensure and regulatory standards. Maintain appropriate patient schedules per established guidelines. Complete timely, accurate documentation to support quality care and reimbursement requirements. Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes. Qualifications Graduate of an accredited Physical Therapy program. Current Physical Therapist license (or eligibility for licensure) in applicable state. Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
