Developing, implementing, and evaluating individualized plans of care. Providing skilled nursing care, education, and disease management. Monitoring patient progress and identifying changes in condition. Collaborating with physicians, therapists, caregivers, and interdisciplinary team members to coordinate care. Educating patients and families on medications, treatments, and disease management. Accurately documenting visits and maintaining timely clinical records. Ensuring compliance with all clinical standards, agency policies, and HIPAA regulations. Advocating for patients while promoting the highest quality of care. Qualifications Current, unrestricted Massachusetts Registered Nurse (RN) license. Minimum of one year of nursing experience in a clinical setting. Home Health experience preferred, but not required. Experience with OASIS documentation is preferred. Experience with Homecare Homebase is a plus. Strong assessment, communication, and organizational skills. Associate's Degree in Nursing required; Bachelor's Degree preferred. Valid driver's license, reliable transportation, and current auto insurance. Ability to travel throughout the Newton service area. Ability to safely lift, push, pull, and assist with up to 50 pounds as needed.
