Home Health Physical Therapist
Location: Kennett Square, PA

We’re seeking a Physical Therapist to deliver one-on-one home-based therapy to patients throughout Chester County. Key Responsibilities

  • Assess, treat, and support patients in their home environment

  • Develop tailored care plans to help patients regain strength, function, and independence

  • Collaborate with an interdisciplinary care team to support holistic patient outcomes

  • Provide education to patients and families to encourage progress between visits

  • Complete accurate and timely documentation within the EMR

Qualifications

  • Bachelor’s degree or higher in Physical Therapy

  • Active PA PT license (or eligibility)

  • Valid driver’s license, auto insurance, and BLS certification

  • Home health experience is welcome but not required—training provided

Schedule & Compensation

  • Full-time: Monday–Friday, 8am–5pm

  • Salary: competitive (based on experience)
