What You'll Do
- Provide skilled physical therapy services to patients in their homes
- Develop and implement personalized treatment plans
- Conduct assessments to determine patient needs
- Educate patients and caregivers on therapy routines
- Document treatment progress and update medical records
- Collaborate with healthcare teams to ensure quality care
- Maintain compliance with health regulations and facility policies
What You Need
- Currently licensed as a Physical Therapist in Texas
- BLS certification (must be obtained upon hire)
- Preferably 1 year of experience in home health, but new graduates are encouraged to apply
- Strong communication and patient care skills
- Ability to work independently and manage a flexible schedule
