Nurse Practitioner

The Nurse Practitioner practices to the full extent of their education, training, and certification. Core functions are outlined in the clinical privileges that correspond with this position. The Nurse Practitioner has prescriptive authority for a panel of patients or programs as indicated by their position. The Nurse Practitioner is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients. Major duties and responsibilities of the position include independently managing a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. They develop and implement an individualized plan to achieve optimal population health and disease management. They perform comprehensive evaluations, including physical examinations, ordering and interpreting diagnostic tests, order interventional procedures, and reviewing patient medical histories in person and through telehealth. They review and manage patient medication regimens for clinical effectiveness, drug selection, dosing contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. They formulate and implement personalized care plans utilizing pharmacological and nonpharmacological interventions. They maintain accurate and timely documentation of pain management and therapeutic changes in the Electronic Health Record (EHR). They collaborate with multidisciplinary teams, including the Patient Aligned Care Team (PACT) providers, pharmacists, and specialists to provide comprehensive pain management. They perform education to patients and their families on pain management strategies, lifestyle modifications, self-care, and Overdose Education and Naloxone Distribution (OEND). They demonstrate leadership and work collaboratively with the interdisciplinary team. They order and interpret labs, tests, and diagnostic studies. They follow care management as outlined in multi-service line agreements. They initiate referrals and consult to specialty services. They assess and adjust the plan of care as needed to achieve optimal patient outcomes. They exemplify the I CARE core values of Integrity, Commitment, Advocacy, Respect, and Excellence.
