Certified Occupational Therapy Assistant (COTA) - Full-Time

Location: Manitowoc, WI
Facility: River’s Bend Health Services
Setting: Skilled Nursing Facility (SNF)

Continuum Therapy Partners is seeking a compassionate and dedicated Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team at River’s Bend Health Services in Manitowoc, Wisconsin. This Full-Time opportunity offers the chance to make a meaningful difference while working in a supportive, collaborative skilled nursing environment. PRN opportunities are also available for qualified candidates.

As a Certified Occupational Therapy Assistant (COTA) , you’ll work alongside an experienced Occupational Therapist to help adult and geriatric patients improve independence, mobility, and overall quality of life through individualized occupational therapy treatment.

• Provide occupational therapy treatments under the direction and supervision of a licensed Occupational Therapist.
• Implement individualized treatment plans designed to improve patients’ functional abilities and independence.
• Monitor patient progress and communicate updates to the supervising Occupational Therapist.
• Complete accurate and timely documentation in accordance with company, state, and federal guidelines.
• Collaborate with physicians, nursing staff, families, and the interdisciplinary care team.
• Educate patients and caregivers on treatment techniques and safety recommendations.
• Deliver compassionate, patient-centered care while maintaining the highest clinical standards.

• Graduate of an accredited Occupational Therapy Assistant program.
• Current Wisconsin Certified Occupational Therapy Assistant (COTA) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Excellent communication, organizational, and teamwork skills.
