Office Location: Gainesville
Coverage area: South Gainesville, Hawthorne, Micanopy,  

Schedule: FT, Monday - Friday 8a-5p. No Weekends or Holidays.

As a RN Case Manager You will: 

Assess/monitor physical, emotional, and psychological needs of patients

Create hospice care plans that align with the patient's wishes and goals

Direct nursing care: administering medications, treatments, and interventions

Provide pain and symptom management

Educate and support the patient’s family and caregivers

Collaborate with interdisciplinary team

Maintain accurate and timely documentation
