This full-time position will primarily work in Brainerd, providing Care Coordination to clients across multiple Northern Pines services and all service locations.Essential Functions include:Attend Integrated Treatment Plan meetings for clients enrolled in Care CoordinationAttend Rule 79 Case Consultation Meetings to facilitate client needsAttend Thursday Integrated Consultation MeetingsAssess clients need to establish Primary Care (Medical), Dental Care and Eye Care. Provide needed linkage and follow-up to linkageConnect with clients to address identified needs on the SDoH. Provide resources and referrals that would meet needsClose that loop on resources and referrals to ensure that client has received the resources and/or engaged in the referred servicesConnect with client to discuss services that are recommended by Mental Health Professional following a Comprehensive Evaluation, Crisis Encounter, Psychiatric Evaluation, ITP meeting or closing of services. Submit the referrals agreed upon by clientAdvocate for clients to help ensure their needs are metAssess client satisfaction with servicesProvide MNSURE Navigation to individuals without insuranceProvide Crisis Resources to clients and assist in connecting to Crisis ServicesProvide Health and Wellness Education and Information to clientsSchedule and attend on-going visits for clients enrolled in Care Coordination ProgramWorking collaboratively with Community Partners – may include travel and working from community sitesManage daily reports in Electronic Health Record and workflow foldersMaintain accurate and timely documentation of services provided to clientProvide outreach to clients regarding alerts received through EAS regarding recent Admit/Discharge from Hospitalization, Child/Teen WellnessQualifications include:High School Diploma or approved equivalent combination of education and experienceTravel is a function of this position; a valid driver's license is requiredCompensation & Benefits include:A starting wage of $22/ hourHealth, Dental, Disability and Life insurance with other additional voluntary options for spousal life, short term disability, accident, hospital indemnity and critical illness plansHealth Savings account with employer contribution401K with employer matchGenerous paid time off (1st-3rd year 19 days/152 hours for full time then increases with longevity!)Nine holidays observed per year as follows: New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, the day after Thanksgiving Day, Christmas Eve Day, Christmas Day and a Weather Float Day.Supportive of time off for a healthy work/life balanceBereavement PayTravel reimbursementPaid Supervision if working toward independent licensureNorthern Pines Mental Health Center is an Affirmative Action/ Equal Opportunity Employer.
