LPN Medication Care Manager - Senior Living

Licensed Practical Nurse (LPN) Medication Nurse responsible for providing safe medication administration, health monitoring, and accurate clinical documentation in a senior living environment. Key Responsibilities Administer medications and treatments according to physician orders, following the Six Rights of Medication Administration. Document accurately in MAR/TAR in real time and support care plan updates. Monitor resident symptoms, report adverse reactions or refusals promptly, and observe physical, emotional, or cognitive changes. Conduct Service & Health Updates (SHUs), transcribe orders, and maintain medication carts with compliance standards. Ensure resident safety through clinical precision, early identification of health changes, and regulatory adherence. Qualifications & Skills Active LPN license in good standing, CPR and First Aid certification. Strong assessment and documentation skills. Preferred experience in senior care or medication administration.
