Outpatient Mental Health Therapist Position at WEST SIDE FED SR.SUP in New York. Full‑time, exempt. Salary $55,000‑$62,000. Job Summary The Social Worker I works autonomously to develop 1:1 relationships with residents, providing comprehensive social work services that enable them to establish and maintain safe, independent lifestyles within the community. Job Duties & Responsibilities Assist residents with: Obtaining and maintaining medical and mental health services, entitlements, and home care Establishing and maintaining family and social relationships Budgeting and income management to ensure timely rent payment Money management Provide: Crisis intervention services Client appointment escorts Individual counseling Assist residents in participating in socialization activities through in‑house and community referrals Follow up with residents and other service providers during out‑of‑facility stays such as hospitalizations and short‑term rehabilitation placements Oversee the discharge planning process, including completion of all necessary discharge forms and applications to other facilities Provide group work services, including recreational, educational, and clinical group work Participate in the intake process for prospective residents as assigned Share resident progress, needs, and problems with staff, guided by the confidentiality policy Complete non‑reportable and reportable incident reports as assigned Maintain competency in software and databases as required Implement emergency procedures as necessary Attend supervision, team, and staff meetings Maintain thorough case records, including case notes, support plans, and psychosocial histories within required time frames Other duties as assigned Required Knowledge, Skills & Abilities Good written and oral communication skills Good organizational ability, time management, and interpersonal skills Desire or interest in supporting an aging population Required Education, Experience & Credentials MSW or MA in human services field with a minimum of two years social work experience Fluency in written and oral English required; bilingual a plus Experience with older adults and people with mental illness and/or substance use preferred Physical Requirements May involve sitting or standing for long periods and heavy computer usage Must be able to stand for extended periods Benefits Overview Full‑time employees receive medical, dental, vision, 403(b) retirement savings, flexible spending accounts, life & disability, short‑term and long‑term disability, employee assistance program, commuter benefits, and more.
