Part-Time and Per Diem (Every other Weekend) | $32.00 per hour

We are seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our Wellness team on a part-time or per diem basis . The LPN will provide hands-on resident care, administer medications, maintain accurate documentation, and collaborate with the Registered Nurse and care team to support residents' health, safety, and well-being.

Responsibilities:

• Provide resident care in accordance with individualized service plans.
• Assist residents with activities of daily living as needed.
• Administer prescribed medications and accurately document all services provided.
• Monitor residents for changes in health, behavior, or functional status.
• Promptly communicate resident concerns or changes in condition to the Registered Nurse and appropriate healthcare providers.
• Assist with developing and updating resident service plans and clinical documentation.
• Respond to resident emergencies, provide first aid, and arrange appropriate medical follow-up.
• Maintain accurate records in communication logs, service plans, and daily care documentation.
• Provide compassionate one-on-one support to residents as directed.
• Help maintain a safe, comfortable, and respectful environment.
• Support the Registered Nurse with staff training, assignments, and other clinical responsibilities as needed.
• Follow all community policies, procedures, and applicable healthcare regulations.
• Promptly report resident incidents and work-related injuries.

Qualifications

• Current and active Licensed Practical Nurse (LPN) license in the State of New Jersey.
• Successful completion of an approved practical nursing program and required licensing examination.
• Ability to work collaboratively in a fast-paced healthcare environment.
• Strong communication, organizational, and documentation skills.
• Compassionate bedside manner and excellent attention to detail.
• Experience in assisted living, senior living, or long-term care is preferred.

Shift:

• 3:00pm-11:00pm
• 11:00pm-7:00am
• Every other weekend

$32 - $32 an hour
