Stepping Up Case Manager - Adult Mental Health Advocate

Carastar Health is seeking an Adult Case Manager for the Montgomery County Stepping Up Program in Alabama. This position involves intensive case management and coordinating services for adults with Serious Mental Illness (SMI). You will collaborate with local stakeholders to implement the Stepping Up model in the community. The ideal candidate will have a Bachelor's Degree in Behavioral Science and two years of relevant experience. Key responsibilities include developing service plans, conducting assessments, and delivering outreach activities.
