Responsibilities: Performs initial and discharge evaluations, sets goals and treatment plan and documents appropriately. Develops treatment plan and performs optimal treatment in a timely manner to meet functional goals. Determines therapy educational needs and provides PRN. Participates as a member of the interdisciplinary team.
